Complete this form and print. Write a check payable to UHH for appropriate amount. Three day (6 hour) classes are $36; two-day (4 hour) classes are $24; one-day (2 hour) classes are $12. ( If you are not a SeniorNet member classes are $46, $34, and $22 respectively.) There are 4 student workbooks, one for Computer Fundamentals 1, one for Excel 2007, one for File Management, and one for all other classes. Each workbook is $15.

If you are not already a member of national SeniorNet, you may join the first day of class. The fee is $40 for individual membership, $60 for couples. The membership lasts for 12 months from date you sign up. Credit cards are accepted.
